    Intermountain Orientation and Badging Information

    Orientation Satus: INACTIVE

    At this time, Intermountain has put Orientation on hold while they collaborate with us to ensure that each new clinician is provided with vital information, system access and anything else that is required to successfully get scheduled for/complete shifts at Intermountain.

     

    If you are a nurse that HAS NOT worked at Intermountain through the Nursa app, please email kristin@nursa.com with expressed interest and our specialist will contact you shortly to discuss qualifications and requirements. If it is determined that you can proceed with the credentialing and orientation process, you will receive an email with further instructions including a date, time and location, as well as what to bring with you.

     

    When you receive the orientation email, please respond quickly to confirm you will be there and bring a photo for your badge with a plain background.

     

    Intermountain Credentialing Items

    Below is an in depth list of credentials and documents required to pick up shifts at Intermountain Healthcare through Nursa. When you have completed the certification or document, search for that specific credential in the Nursa App and upload it to your Nursa profile. 

     

    Drug Screen & Drug Screen | You will receive it though Yardstik. 

    Department Orientation Checklist | Nursa will send this to you via email | Print out to take to in person orientation 

    Pre Boarding Documents | Nursa will send this to you via email | Print out to take to in person orientation 

    OSHA Medical Questionnaire | Nursa will send this to you via email | Print out to take to in person orientation 

    Intermountain Orientation Booklet | Nursa will send this to you via email | FOR REVIEW ONLY

    ACLS and/or BLS & Any Other Certifications | Upload through App

    Resume | Upload through App

    Driver’s License | Upload through App

    Immunizations | Upload through App

    1. Tuberculosis screening requirement. One of the following is required:

    (a) 2-step TST (two separate Tuberculin Skin Tests, aka PPD tests) is placed no sooner than seven (7) days apart and no longer

    than twelve (12) months apart. The last TST should be completed at the time the Worker begins their work assignment at an

    Intermountain Healthcare facility.

    (b) One (1) QuantiFERON Gold blood test with negative result.

    (c) One (1) T-SPOT blood test with negative results.

    If previously positive to any TB test, the Worker must complete a symptom questionnaire and have a chest x-ray read by a radiologist

    with a normal result. If chest x-ray is abnormal, the Worker needs to be cleared by their physician or local health department before

    beginning their work assignment at an Intermountain Healthcare facility.

     

    1. Measles (Rubeola), Mumps and Rubella requirement. One of the following is required:

    (a) Proof of two (2) MMR vaccinations.

    (b) Proof of immunity to Measles (Rubeola), Mumps, Rubella through a blood test.

     

    1. Tdap requirement:

    (a) Proof of one (1) Tdap vaccination after age ten.

     

    1. Varicella (Chicken Pox) requirement. One of the following is required:

    (a) Proof of two (2) Varicella vaccinations.

    (b) Proof of immunity to Varicella through a blood test.

     

    1. Flu Vaccination requirement:

    (a) Proof of current, annual influenza vaccination.

     

    1. Hepatitis B. The Hepatitis B series should be offered to anyone who is at risk for an occupational exposure, which is defined as

    someone with a reasonably anticipated skin, eye, mucous membrane, or parenteral contact with blood or other potentially infectious

    materials that may result from the performance of their duties. One of the following should be performed:

    (a) Documentation of three (3) Hepatitis B vaccinations and blood test with “Reactive” results.

    (b) Documentation of three (3) Hepatitis B vaccinations given more than 8 weeks prior to start date with no documented blood test

    results (no blood test is required, but a baseline titer should be run immediately if the person has a significant exposure to blood

    or body fluids).

    (c) Blood test with “Reactive” result.

    (d) Documentation of six (6) Hepatitis B Vaccinations with blood test result of “Not Reactive” (this person is considered a “NonResponder”).

    (e) Documentation of two (2) Heplisav B vaccinations and blood test with “Reactive” result.

    (f) Documentation of two (2) Heplisav B vaccinations with no documented blood test results (no blood test is required, but a

    baseline titer should be run immediately if the person has a significant exposure to blood or body fluids). 

     

    1. COVID-19 Vaccine:

    (a) Proof of current vaccination.

     

    Skills Assessment and Modules:  

    The modules and skills assessment form will need to be completed before you arrive for in person orientation. You will need to bring the Skills Assessment with you to Orientation to get it signed by the educator.  Please click the link below that applies to you.  This link will redirect you to Intermountain's website where you will be able to download the skills assessment form as you go through the modules listed on that webpage.
